ONGOING SERVICE COORDINATORS–Immediate caseloads available!
Responsibilities include:
Coordinating and monitoring all early intervention services in accordance with the IFSP, attending IFSP meetings, coordinating transitions to CPSE, communicating with families and other early intervention disciplines, documenting and maintaining written records and reports regarding OSC activities, and following mandatory guidelines. May work from home, but must be available for home visits and to attend meetings.
Qualifications include:
- Bachelor’s Degree in Health or Human Services
- Experience in Early Intervention or case management a plus
- Organized, detail oriented
- People skills a must
- Bilingual a plus
- Fee-for-service position (part-time or full-time)
- Flexible hours
- Will train
- Must be organized and able to meet deadlines

-Areas include: Long Island, Queens, Brooklyn, Staten Island, Manhattan
